Gift of Carl
Instrument Scholarship Form

    • This form should be completed by the music teacher, not a parent or student.

    • Submitting this form indicates approval of the scholarship by the student's Music Teacher and Principal.

    • We understand that this request will be fulfilled based on the availability of the instrument(s) requested.

    • The instrument may be used by the student for as long as that student participates in the school music program.

    • If an instrument is granted on a scholarship to this student, the school agrees to monitor its use.

    • If the student no longer uses the instrument and the school program does not have a need for this instrument, the instrument may be returned to The Gift of Carl Foundation.

    • The Gift of Carl Foundation has the right to refuse any returned instrument.
